Webinar: How to Use the ACO Model to Develop a Population Health Strategy

When Atrius Health decided to participate in the Pioneer ACO program, we saw it as an opportunity to evolve a population health approach for all of our Medicare-eligible patients. Using a payer-blind, population health approach, Atrius Health analyzes integrated claims and EHR data to stratify the population, detect gaps in care and identify opportunities to improve outcomes.

We are excited to announce that on June 25, 2015, our own Emily Brower, Executive Director of Accountable Care, and Dr. Richard Lopez, Chief Medical Officer will host a Healthcare Web Summit about how we have used the Pioneer ACO as a platform to develop our population health strategy.
