Webinar: How to Use the ACO Model to Develop a Population Health Strategy

| Posted On Jun 22, 2015 | By:

When Atrius Health decided to participate in the Pioneer ACO program, we saw it as an opportunity to evolve a population health approach for all of our Medicare-eligible patients. Using a payer-blind, population health approach, Atrius Health analyzes integrated claims and EHR data to stratify the population, detect gaps in care and identify opportunities to improve outcomes.

We are excited to announce that on June 25, 2015, our own Emily Brower, Executive Director of Accountable Care, and Dr. Richard Lopez, Chief Medical Officer will host a Healthcare Web Summit about how we have used the Pioneer ACO as a platform to develop our population health strategy.

Following the webinar, participants will be able to:

  1. Review Atrius Health’s Pioneer ACO results, experiences, insights, opportunities and challenges
  2. Examine how Atrius Health leveraged its Pioneer ACO platform
  3. Consider how to launch a population health strategy with an accountable care platform
  4. Ascertain how to improve the care of geriatric patients with triple aim accountability
  5. Understand how to create a clinically integrated system of care
  6. Identify how to develop clinical best practices for medical management and care coordination
  7. Explore models for practice-plan integration
  8. Engage in interactive learning through online question submission, attendee feedback and opportunity for follow-up questions and networking with attendees, faculty and other professionals through a dedicated LinkedIn group

At Atrius Health, we believe that a population health approach is the best way to ensure high quality care for our patients, and we look forward to sharing our insights with you. Please visit Healthcare Web Summit’s website here to register.
